Our web-based referral system provides multiple, clear referral options to referring practices while adhering to industry best practices and regulatory requirements.
These online forms will provide quality assurance to both referring practices and our medical teams – ensuring all pertinent data specific to an individual patient’s case has been shared efficiently and in a legible, printable format.
Our referral form details the necessary documentation we require about your client and patient. After the referral is received and reviewed, we will contact your client via the phone number recorded on the referral to schedule an appointment.
Our intent is to always provide you with a case summary within 24 hours of discharge. If you have not received patient records, please contact our mainline at 709-221-7838.
